Thank you for applying! ********** PACE of Southwest Michigan Also, Area Agency on Aging has partnered with Hospice at Home and Lakeland Regional Health System to bring PACE to the community. Program of All Inclusive Care for the Elderly (PACE) is a program that provides health and social services for the elderly in our community. This program is for elderly who are nursing home eligible but wish to remain in their homes. PACE works with the elderly and their caregivers to provide needed services to make this happen. PACE provides services in a day health center and in the participants' home as needed. At the Center, the participant can see the physician, therapist, social worker, nurse, activities coordinator, dietician and more. The PACE team works with the participant to develop the best individualized plan of care. PACE also provides transportation between the home and the Center. Area Agency on Aging, Lakeland and Hospice are excited to bring this needed program to the elderly in our community. The PACE goal is to improve the independence and quality of life for our elderly and to assist their caregivers in the coordination of needed services.
