Care Management Department: 

The Care Management department is hiring!  We are looking for one full-time Quality Manager.

Care Managers must be Michigan-licensed or limited license Social Worker (LLBSW, LBSW, LLMSW, LMSW), or a Registered Nurse or Bachelor of Science in Nursing (RN or BSN). Full job descriptions are listed below.

Care Managers travel throughout Berrien, Cass, and Van Buren Counties. Each Care Manager works as part of a team including one Nurse and one Social Worker to complete a comprehensive assessment of each applicants needs. Care Managers meet with potential clients in private homes/apartments, licensed settings such as Adult Foster Care Homes/Homes for the Aged/Assisted Living facilities, and hospitals and in nursing homes for residents that want to return to community living. Care Managers determine program eligibility to help meet needs. Develop, update, and implement an individualized person-centered Care Plan which addresses the specific and unique needs of each client. Computer skills, long-term care experience, and knowledge of community resources helpful. Serving adults of all ages and all income levels. See job descriptions below.
